Draw Terms & Conditions
- Eligibility: The $200 gift certificate draw ("the draw") is only available to patients of the Orleans location of Ottawa Physiotherapy and Sport Clinics.
- Entry: To be entered into the draw, individuals must submit an entry form provided by Ottawa Physiotherapy and Sport Clinics.
- Draw Date: The draw will be held on June 14th, 2024.
- Notification: The winner will be notified by email within 7 days of the draw date. If the winner cannot be contacted or does not claim the prize within 14 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner.
- Prize Acceptance: The winner may be required to sign a release form consenting to their inclusion in marketing materials for Ottawa Physiotherapy and Sport Clinics.
- Decision: The decision of Ottawa Physiotherapy and Sport Clinics regarding any aspect of the draw is final and binding and no correspondence will be entered into.
- Agreement to Terms: By submitting an entry form for the draw, entrants agree to be bound by these terms and conditions.
- Amendment: Ottawa Physiotherapy and Sport Clinics reserves the right to amend or alter these terms and conditions at any time and for any reason.
- Exclusion: Employees of Ottawa Physiotherapy and Sport Clinics and their immediate families are not eligible to participate in the draw.
